The equations
eGFR = 142 × min(Scr/κ, 1)^α × max(Scr/κ, 1)^−1.200 × 0.9938^age × 1.012 (if female)
κ = 0.7 female, 0.9 male · α = −0.241 female, −0.302 male
MDRD: eGFR = 175 × Scr^−1.154 × age^−0.203 × 0.742 (if female)
Cockcroft-Gault: CrCl = ((140 − age) × weight kg) ÷ (72 × Scr) × 0.85 (if female)
Scr in mg/dL · eGFR in mL/min/1.73 m² · CrCl in mL/min
Glomerular filtration rate describes how much blood your kidneys filter each minute. Measuring it directly is impractical, so it is estimated from serum creatinine — a waste product of muscle turnover that healthy kidneys clear steadily. The 2021 CKD-EPI equation is the current standard in most guidelines because it removed the race coefficient that earlier versions used.
What can throw the estimate off
Because creatinine comes from muscle, a very muscular person can look worse than they are and someone with low muscle mass can look better. Large protein or creatine intakes, some medicines, acute illness, pregnancy and rapid changes in kidney function all reduce reliability. A single result never establishes chronic kidney disease: that needs repeat testing over at least three months alongside urine albumin, interpreted by a doctor.
